Facility Training
**SPECIFIC CLASSES FOR EACH DAY MAY CHANGE**
St. Joseph Hospital, Mullen Building, Board Room, 19th and Franklin, Denver, Colorado (go to www.mapquest.com
to print a map). Parking is available in the parking garage on 19th and Franklin for a small fee; or you
can find free parking several blocks to the north of the building. The Mullen Building is
north of the hospital's twin towers, on the west side of Franklin. Maximum class size is 24.
We ask that all cell phones be turned off and phone calls be made only during the
breaks to avoid disruption to other participants. Participants must register for the
complete four-day course. Partial registration will not be accepted. The deadline for
registration is August 18, 2008. Total cost is for the course is $499 which includes
class materials, lunches and refreshments. The discounted cost to CGS member is
$449.

Students
must register for the complete four day course. Partial registration will not be accepted. The deadline
for registration is August 18. To qualify for the discount, the sponsoring organization must
have an organizational membership. Registration fee includes class materials, lunch and refreshments.
Refund policy: All cancellations must be received in writing five days prior to the session. A $100
processing fee will be charged for all cancellations. Refunds will be made within thirty days of the course.
There will be a $35 charge for dishonoring or returned checks.
CGS Membership Discount: CGS organizational members receive a 10 percent registration discount for this course.
CGS dues must be current at time of registration. You may join or renew your CGS membership at time of
registration by including the annual membership fees in your payment, and completing the membership application
