Fill Out a Voluntary Contributions Schedule
- If filing a paper return, attach and complete form DR-0104CH
- If filing an online return via Revenue Online, complete the Contributions page immediately prior to the submission page
- If using third-party tax filing services or software, ask or search for ‘Colorado Voluntary Contributions’ or similar
- On Line 16 of the paper DR-0104CH, or the Revenue Online Contributions page, or the appropriate field on third-party software, enter:
- Name: Colorado Gerontological Society
- 11-Digit Secretary of State Registration Number: 20093006522
- Enter a dollar amount to donate to CGS, up to the total amount of your refund
