“What Matters to Me?” Workbook is designed to help people with serious illness get ready to talk to their healthcare team (doctor, nurse, social worker, etc) about what is most important to them. The Workbook is not about making specific medical decisions. It is about thinking about what matters most to you — and sharing your goals and preferences with your healthcare team. Then together you can choose the kind of care that is right for you.

On National Health Care Decisions Day, we will review the What Matters to Me Workbook. The Workbook was developed by the Conversation Project, an initiative of the Institute for Health Care Improvement.
