Program of All-inclusive Care for the Elderly (PACE)

The Program of All-inclusive Care for the Elderly (PACE) is a program designed to offer comprehensive health services and in-home care, including supportive services to help individuals maintain independence in the community for as long as possible. It is only open to dual eligible individuals, receiving both Medicare and Health First Colorado (Medicaid) who are over age 55 (including those disabled Medicare beneficiaries between 55 and 65).

PACE is funded as a capitated health plan through Medicare for Medicaid clients. The program is subcontracted out for the full continuum of care. As of 2017, four providers with limited services areas provide PACE:

County   PACE Organization Phone Number
Adams, Arapahoe, Broomfield, Denver, Jefferson  Innovage Greater Colorado PACE 720-974-2411
Boulder, Weld (Southwest)  TRU Community Care/TRU PACE 303-665-0115
Pueblo  Innovage Greater Colorado PACE 719-553-0400
El Paso  Rocky Mountain PACE 719-314-2327
Delta  Senior CommUnity Care/VOANS PACE 970-835-8500
Montrose  Senior CommUnity Care/VOANS PACE 970-252-0522

As a Medicare Advantage plan, PACE is also a Special Needs Plan (SNP) and individuals who enroll in it are required to use their provider’s network of physicians, hospitals, nursing homes, home care agencies and other professionals. This also means there is a formulary for drug benefits for PACE.

The Health Care Team at the administering agency must coordinate health care services, assess the participant’s needs regularly, develop and monitor a care plan, provide direct services, coordinate outpatient services and inpatient hospitalizations, contract with specialty physicians and providers and support community-based living where appropriate. Each participant is assigned a primary care staff member who is the main contact for that participant’s care. The agency must operate at least one Adult Day Center (with hours 8:00 am to 4:30 pm M-F).
