Dental Flyer - Extra Funding

Limited Time Grants Available
The Colorado Gerontological Society has received extra funding for dental services for seniors that must be spent by June 30, 2016. Submit an application today!

Through a grant from the Colorado Department of Health Care Policy and Financing and supported by other community grants and donations, CGS administers the Senior Low Income Dental Program to provide limited financial assistance to Denver metro area seniors for basic dental care.

Applicants must be a minimum age of 60 years old and must reside in the Denver Metro area (Adams, Arapahoe, Broomfield, Clear Creek, Denver, Douglas, Gilpin and Jefferson counties). Individuals who receive Medicaid and/or have private insurance are not eligible. Applicants are screened for social and economic need in accordance with the Senior Low Income Dental Guidelines guidelines (available upon request). Services will
be prioritized to those in the greatest need. Applicants will choose their own dentist, who must be licensed to practice in Colorado and must agree to accept a Dental Program grant as payment for services.

INDIVIDUALS WHO RECEIVE MEDICAID OR HAVE PRIVATE INSURANCE ARE NOT ELIGIBLE FOR THE PROGRAM.  INDIVIDUALS ON MEDICAID SHOULD CALL 855-225-1729 FOR MORE INFORMATION ABOUT QUALIFYING FOR DENTAL CARE. THOSE NOT ON MEDICAID, MAY CALL 303-333-3482 FOR REFERRALS TO OTHER COMMUNITY RESOURCES IF THEY ARE NOT ELIGIBLE FOR OUR PROGRAM.
For those approved for a grant, the Dental Program will provide an initial grant to have an exam and x-ray done to evaluate the applicant’s needed services. The dentist will forward the proposed plan of treatment to CGS, who will then evaluate it for a final grant for services. No payments will be made to patients or before all work has been completed. In addition the Program will not pay for ANY services performed before explicit written authorization is received by the dentist. The program does not cover crowns, root canals, fixed bridges or implants, but will cover full and partial dentures, fillings (amalgam and composite), extractions, relines and cleanings. The full fee schedule is available upon request.

Individuals are encouraged to apply for dental services. More information on the programs or information for providers interested in signing up for the programs are available at 303-333-3482.

To apply, please complete a Senior Low Income Dental Application and submit along with all required documentation to The Society. Applications may be submitted by fax to 303-333-9112 or by mail to Senior Low Income Dental Program, 1330 Leyden St, Suite 148, Denver, CO 80220.
